NEW TEAM REGISTRATION INSTRUCTIONS FOR PARENT REP ONLY

REGISTRATION IS CLOSED FOR THE 2018 SEASON. REGISTRATION FOR 2019 SEASON OPENS IN OCTOBER.

STEP 1: CREATE YOUR TEAM

NOTE: Returning teams can access information from last season by clicking here.

Your team's roster is entered electronically here by your parent rep.

  1. Contact the webmaster through the contact page and request to form a new team.
  2. Once you have received confirmation of your team page creation, login using the credentials you submitted.
  3. Enter your Team Name, Parent Rep, and Coach
  4. Click Save
  5. Instruct your parents to complete the Free Agent form and put YOUR TEAM NAME in the Message box.
  6. Once your Players have been entered, review to make sure the Player information is correct and complete the Checklist below.
  7. Submit your Registration Packet to the Registrar before the registration deadline.

STEP 2: ELECTRONIC SIGNATURE FOR LIABILITY WAIVER AND CODE OF CONDUCT

Waivers and Code of Conduct are required for EACH participant. This includes Adults helping on the field during games, field maintenance, and Skills Competition. Please forward these instructions to your parents. Once they are signed, have them forward the email confirmation to you for your records.

INSTRUCTIONS:

NOTE: Waivers will be available in January.

  1. Click here to access the LEAGUE LIABILITY WAIVER and CODE OF CONDUCT
  2. Read through the document and Initial each section
  3. Click "ADULT and MINOR(S)" to sign for EACH Minor participating in Sansei Baseball League. NOTE: If you will be participating in multiple divisions, please indicate each Team/Division you will be active with in the field provided. (See Fig. 1 below)
  4. IMPORTANT: Each additional Adult participant will also need to fill out the "ADULT" Waiver and indicate the Team(s)/Division(s) they will be participating with. This includes games (coaches and assistant coaches), field maintenance, and Skills Competition.(See Fig.1 below)
  5. Once your waiver is signed, an verification email will be sent to you. IMPORTANT: Your waiver is NOT complete until you verify your email address. If you do not receive a verification email, please check your SPAM folder or Contact the webmaster.

EXAMPLE SCENARIO:
Your family has 2 kids participating on different teams. By clicking "Adult and Minor(s), your waiver covers You and BOTH kids. If your spouse will also be participating, he/she will need to fill out the "Adult" waiver.

NOTE: If you are participating on multiple Teams and Divisions, be sure to indicate them in the field provided.


STEP 3: REGISTRATION FEES

Fee is $75 per player PLUS additional fees listed below in Step 3B. Sibling discounts apply as follows:

  • Youngest: $75
  • Older/Oldest: $65
  • Oldest: $55
  • Maximum $195 per family

NOTE: Only list siblings who are Sansei League members

STEP 3B: ADDITIONAL MANDATORY FEES

  • $10 per player - Japanese Cultural Institute (GVJCI) Matsuri raffle tickets. Attached coupons good for use on food and games at the festival held in June.
  • $20 per player - SBL Scholarship Opportunity Draw tickets. Fees collected go directly to selected students who meet the SBL Scholarship requirements.

NOTE: Parent Reps will be responsible for collecting fees from each player and submitting ONE PAYMENT for each team.


STEP 4: REGISTRATION CHECKLIST

  1. Registration Packet
    1. Print Completed Roster (2 copies)
    2. Copy of Concussion Protocol Certificate - COACHES ONLY
    3. If paying by check, include ONE CHECK for the entire team. If your team has paid online, please print and submit the receipt.
  2. Fees - NOTE: ONE payment for ENTIRE team
    • By Check: Write ONE check for the entire team and include in your Registration Packet.
    • Pay Online: To pay online, use the Contact Page to request an invoice. Please include your team name and division. The invoice will be sent via Paypal.
  3. League Liability Waiver and Code of Conduct - E-Signature required for EACH participant (See STEP 2.)
  4. Concussion Protocol Certificate - COACHES ONLY
  5. Consent to Treat - Signed and submitted to Coach or Parent Rep (NOTE: Not in Registration Packet)
  6. Player Transfer - Signed and submitted to Division Commissioner (NOTE: Not in Registration Packet)

*IMPORTANT*: Please double check the entire package to ensure it is complete. Incomplete packages will NOT be accepted.
